Job Description

Part-Time 40 hrs per payperiod AM Bachelors Degree Job Summary: The Case Manager utilizes clinical expertise, discretion, and independent judgment to assessing, providing, coordinating and monitoring healthcare services for designated inpatients. Ensures medical appropriateness criteria (intensity of service and severity of illness) are met for level of care provided and is documented from admission through discharge. Practices in a collaborative health care environment to oversee implementation of a well-thought-out interdisciplinary plan of care with an individualized discharge plan that is comprehensive and best meets the continuing health care needs of the patient. Reevaluates plan of care and ensures continued appropriateness based on the patients changing needs and condition. Maintains working knowledge of specialized case and utilization management methodologies and practices; applies concepts to every-day practice. Serves as a role model to other members of the care management team and mentors staff in care management principles and practices. Demonstrates ability to be an effective change-agent for implementation of new processes to achieve desired outcomes/performance measures. Performs other duties as needed. Job Specifications: Bachelor of Science in nursing required. Enrolled in a Bachelor's program with quarterly reports acceptable. Three years of case management, utilization review, or discharge planning experience required. Working knowledge of regulatory requirements and accreditation standards required. Active California registered Nurse (RN) license required. Basic Life Support (BLS) certification required. CCM or ACM certification required within two years of hire. Other certifications as required in department specific Plan for Providing Care. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; perform basic math functions; manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision. *HJ*HJJD *SH*JD *MH*JD *MW*
