Job Description

Full-Time PM Bachelors Degree CA RN license CPR/BLS Job Summary: The Case Manager ? Emergency Department (ED) utilizes clinical expertise, discretion, and independent judgment to apply specialized emergency medicine case management and care coordination methodologies and practices in a tertiary care facility with level one trauma designation. Focuses on the needs of Loma Linda University Medical Center (LLUMC) and Loma Linda University Children's Hospital patient population in partnership with the ED physicians, staff, and inpatient providers. Facilitates patient safety, flow, strategies and outcomes that enhance continuity of quality care delivery. Works creatively within fiscal realities to facilitate safe discharge from emergency care and transition patient into the continuum of care arena as needed. Reviews cases on observation status and/or with prolonged length of stay. Makes certain patients converted to inpatient admissions meet severity of illness and intensity of service criteria. Performs other duties as needed. Job Specifications: Bachelor of Science in nursing required. Enrolled in a Bachelor's program with quarterly reports acceptable. Three years of case management, utilization review, or discharge planning experience required. Working knowledge of regulatory requirements and accreditation standards required. Active California registered Nurse (RN) license required. Basic Life Support (BLS) certification required. Other certifications as required in department specific Plan for Providing Care. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. *HJ* HJBB *MH*BB *SH*BB
